Picture Order Form-  

MHSAA Physical Form – MUST be on file in the High School Athletic Office prior to first practice/tryouts. The form MUST also be signed by BOTH the parent/guardian AND the student-athlete before it is considered to be valid.

Student Transportation Form – Must be on file with the COACH prior to the team’s away contest when school transportation is not available.

Sports Participation Fees- Must be paid to the High School Athletic Office prior to the first competition of each sport.

Visitor & Volunteer Consent Form- Must be completed and on file with Central Office annually.

2022-23 Athletic Handbook

Student-Athlete Handbook Form – Must be on file in the High School Athletic Office prior to first practice/tryouts. It only needs to be submitted one time during a student’s athletic career at DKS.  

Hall of Fame Nomination Form -  Nomination forms must be sent to the Athletic Department by 12 noon on the second Monday in November.
